MJC "How To Complete" Family Forms Videos  
Affidavit of Indigency ("Fee Waiver") 
Used to request a waiver of court filing fees and Sheriff's process service fees
Stipulation to Change (FA-604A) ("Stipulation")
Used when both parents agree to make changes to their current family court orders (for example, custody/placement, child support. arrears balances, tax exemptions);  
Notice of Motion and Motion to Change (FA-4170)
​ ("Modification Motion")

Used when one parent wants to request a change and the other parent does not agree; Once filed, this form will result in a court date being issued by the Family Court Commissioner's Office.
Financial Disclosure Statement (FA-4130) ("FDS")
Used most commonly in a divorce/legal separation to disclose all assets and debts of both parties;
